Medical Yoga Therapy

Service Description

Scope of Services: * This is (1) Medical Yoga Therapy session 2.5 that includes a yoga therapy assessment, a yoga therapy practice, a yoga therapy home regiment, and follow-up session; in the totaling amount of $250.00 Client Responsibilities: * Completed medical take home package. Attend one therapy session with Lizbeth Vazquez. Active participation and communication with Yoga Therapist during practice and follow-up session. No phones allowed during practice unless it’s a medical emergency. Follow take home regimen for a week and follow-up with yoga therapist. Confidentiality: * Assuring the client that information shared during sessions will be kept confidential, with exceptions for situations where there is a risk of self-harm and/or others. Informed Consent: * Completion of informed consent to participate in yoga therapy. This includes understanding the nature of the practices, potential risks, and benefits. Payment and Fees: * This is (1) Medical Yoga Therapy session 2.5 In the totaling amount of $250.00 Payments are to be directed and handle by Lizbeth Vazquez owner of YogicMethods. Cancellation Policy: * A 24-hour notice of cancellation is needed to reschedule your appointment. Should you miss to give your noticed you will be charged a late fee of $50.00. Address your cancellation and/or rescheduling needs by texting and/or calling Lizbeth directly to. Please re-schedule your appointment should you not be feeling well. Code of Conduct: * Respectful communication, appropriate behavior during sessions, and the importance of creating a safe and inclusive space is a priori. Let us work together to support your Yoga Therapy Session and create a container for awareness and insight. Please refrain from using your cell-phones during class and please try to remain in the session the entire time. Professional Boundaries: * Emphasizing that the therapeutic relationship is for the purpose of yoga therapy and not a substitute for other professional relationships. Duration and Termination: * The duration of the agreement is for 2.50 hour and a half. Should code of conduct and professional boundaries be crossed, at any moment either party can terminate the therapeutic relationship. Client Rights: * The client has the right to ask questions at any time during sessions, receive information about the therapy process, and provide feedback.
